Any comments you submit to us through the Site, such as remarks, suggestions, ideas, graphics, or other information become and remain the exclusive property of Connecticut Children’s Medical Center, even if these Terms & Conditions or any other agreement you have entered into with us is later terminated or amended. This means that we do not have to treat any such submission (including, but not limited to, product or advertising ideas) as confidential. We will not pay you or anyone else for any information that you provide which is used by us. Additionally, you acknowledge that you have full responsibility for any such submission you make, including its legality, reliability, appropriateness, originality, and non-infringement.

What is MyChart Medical Record Access?
MyChart Medical Record Access is an Internet application that enables a patient to have secure web-based access to portions of their Connecticut Children's Medical Center electronic medical record, and allows a secure, convenient communications channel with Connecticut Children's for non-urgent messages. Use of this service is optional. Your login name and password are key to protecting the confidentiality of your information. It is your responsibility to keep your password confidential. It is only known by you and can be changed online at any time by you. Your password is not known by anyone at Connecticut Childrens' Medical Center.

Secure Message Guidelines
Please observe the following guidelines for messages sent through MyChart:

  1. Please DO NOT SHARE your password with anyone.
  2. Please do not send any messages requiring urgent attention.
  3. Please note that messages may be viewed by other health professionals and others who work with your physician.

Your Responsibilities
By accessing or using MyChart you agree to be bound by all the terms and conditions of this Agreement and the MyChart Privacy Statement as posted. We may modify this agreement and the privacy statement at any time, and you agree that such modifications are effective immediately upon posting of the modified version. You agree that MyChart is a communication service offered as a convenience to Connecticut Children's patients and families. You understand that it provides you online access to portions of your Connecticut Children's medical record and that you are solely responsible for any sharing of its content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate department as necessary for handling, and therefore Connecticut Children's employees, other than your healthcare provider may be involved in addressing your request. Therefore use of MyChart may not be appropriate for very sensitive information you wish to discuss directly with your provider. You understand that you may receive messages, test results and other communications delivered via MyChart. Therefore, you may not receive other types of communications from Connecticut Children's in these areas.

Discontinuing Use of MyChart Medical Record Access
You can disenroll from your use of MyChart Medical Record Access at anytime by completing the appropriate online request to disenroll or by calling your physician office. If we determine at our sole discretion that you have violated this agreement and/or abused the use of this service, we reserve the right to discontinue your use of this service. You will be notified if we discontinue your MyChart Medical Record Access service.

This Site may provide links or references to other sites. Connecticut Children's Medical Center is not responsible for information, products or services obtained on such other sites and is not liable for any damages arising from access to such Sites. Any links to other sites are provided merely as a convenience to the visitors of this Site and any inclusion of such links in this Site does not imply an endorsement or warranty of the linked sites or their content, products or services. The Terms & Conditions stated in this policy apply only to this Site and the products and services we offer. Visitors to this Site are cautioned that linked sites are governed by their own use and privacy policies, so visitors linking to such sites should make sure to visit the appropriate pages of those sites to determine what policies will apply. Any legal actions, suits or proceedings arising out of these Terms & Conditions will be brought in a state or federal court located in the State of Connecticut. In addition, you agree to submit to the personal jurisdiction and venue of such courts. You acknowledge that monetary damages for improper use of this Site or other breach of these Terms & Conditions may be inadequate. Therefore, Connecticut Children’s Medical Center is entitled to seek equitable relief, including injunctive relief against you, in addition to all other remedies.

Separate Agreements
You may have entered into other agreements with Connecticut Children’s Medical Center. Those agreements are separate and in addition to these Terms & Conditions. These Terms & Conditions do not modify, revise or amend the terms of any other agreements you may have with Connecticut Children’s Medical Center.

If any of these terms shall be deemed invalid, void or for any reason unenforceable, that term shall be deemed severable and shall not affect the validity and enforceability of any remaining terms.
